Another fatality has occurred in the mining industry, and this one occurred when a worker was thrown from the basket of the man lift he was operating.
In looking at this fatality, two things are obvious: ALWAYS have your fall protection attached to the mid-rail connection point of the basket to prevent yourself from being ejected, and do not tram the lift with the boom raised above horizontal.
This operator made two simple mistakes that resulted in the loss of his life; let’s not make the same mistakes ourselves.
